1. Mark 1st and 5th metatarsal areas
2. Place foot in box non-weight bearing.
3. Rotate box to obtain neutral position.
4. Make sure back, thigh, and leg are all at 90 degree angles. Seat patient forward to the front edge of the chair. Press down on top of knee.
5. Press toes in to make sure that the 1st metatarsal, the 5th metatarsal and the heel are all at equal depth.
6. Mark the 1st and 5th met points into the foam for our reference.
7. Verify equal depth. The 1st metatarsal, 5th metatarsal, and the heel must all be the same depth.
8. Enclose all paperwork with your order including both the Shoe Order Form and Custom Insert Order Form.
9. Place a prepaid UPS Label on the outside of the box and fill out the checklist and requested information. Then, hand off to a UPS driver, dropbox, or UPS Store for shipment to Dr. Comfort Lab. PLEASE NOTE YOUR TRACKING NUMBER FOR YOUR RECORDS.
